Description
  • Conduct patient evaluations in both inpatient and outpatient facilities, crafting personalized treatment plans
  • Implement physical therapy modalities targeting mobility enhancement, strength building, balance improvement, and functional restoration
  • Cooperate closely with nurses, physicians, case managers, and multidisciplinary rehab teams to maximize patient outcomes
  • Guide patients and their families in understanding therapeutic objectives, discharge processes, and at-home exercise routines
  • Maintain accurate documentation of patient evaluations, treatments implemented, and progress made in compliance with facility and regulatory standards
  • Engage in care team meetings and interdisciplinary discussions to assure coordinated patient care
Education
  • Doctoral degree in Physical Therapy (DPT) or an equivalent qualification from an accredited educational institution
Certifications
  • Valid state licensure as a Physical Therapist
  • CPR certification is preferred
  • Additional specialized certifications (e.g., in neurology, geriatrics) are beneficial but not mandatory
Skills
  • Prior experience is highly desired
