Medical Associates is hiring a full time Physical Therapist to join our team. Position Details: Develop a treatment plan for patients with physical limitations or impairments based on consultation with the patient, and in coordination with referring providers when involved, as well as other medical staff. This will include review of the medical history and referrals, diagnosis of movement dysfunction, and documentation of a plan of care with clearly outlined goals and progression. Administer treatment program per plan of care in coordination with referring providers instructions, clarifying these instructions where required. Maintain ongoing communication with referring providers and medical staff on patient care issues. Evaluate charts and reports on patient's progress; maintain current and accurate patient documentation. Bill patients according to established procedures and communicate with third-party payers and physicians for reimbursement of services. Comply with Clinic policy and procedures, care protocols and utilization. Educate patients and families in proper care and use of various support and exercise programs.
