POSITION SUMMARY

The Targeted Case Manager supports integrated health and behavioral health care management for clients involved in a large, cutting-edge integrated behavioral health and primary care initiative at Catholic Charities' Institute for the Hispanic Family in Hartford, CT.

Works with CCBHC integrated care team to ensure that the client's behavioral health and addictions treatment, primary/specialty health and social service needs are met through integrated care. Provides psycho-social rehabilitation services to a population of clients with Severe Mental Illness (SMI) or Co-occurring Disorder. Teams with clinicians, clinical director, and psychiatry staff to identify clients in need of TCM services. Teams with clinician to assess client's daily living skills and develops psychosocial rehabilitation service plan addressing daily living skills in health/nutrition, substance abuse, mental health, personal hygiene, personal finances, social relationships and employment. Provides direct face-to-face skills building to the client in the home and community. Connects client with other appropriate peer support and community-based services, as needed. Teams with other staff in co-facilitating psycho-social rehabilitation groups addressing social skills, vocational skills, health and wellness. Administers national evaluation tools as necessary.

GENERAL DUTIES AND RESPONSIBILITIES

  • Provides psycho-social rehabilitation services to a population of clients with Severe Mental Illness (SMI) or Co-occurring Disorders.
  • Teams with clinicians, clinical director, and psychiatry staff to identify clients in need of TCM services.
  • Teams with clinician and family members and natural support to administer a comprehensive assessment to assist clients in gaining access to needed medical, social, educational and other services, including coordinating a plan of services, monitoring services and activities to ensure that the plan continues to meet the wants and needs of the individual.
  • Conducts planning with the client around their goals and needs and participates in treatment planning conferences with the client, the client's other providers and any natural supports.
  • Conducts referral and related activities to help the client obtain needed services with providers and other agencies to obtain services which addresses the individual's needs and helps them achieve the goals documented in their recovery plan.
  • Provides direct face-to-face psychosocial skills building through role modeling and teaching with the client in the home and community. Connects client with health, vocational, housing, social and behavioral health services and supports as needed.
  • Provides client outreach through home visits or via phone to ensure medication adherence and follow through with important behavioral health and primary/specialty health care needs. Conduct outreach visits to client's home, hospitals/inpatient facilities and to community sites to engage client and provide services. Attempt to follow up with client within 24 hours of hospital/inpatient discharge to reengage him/her in aftercare services.
  • Supports the Community outreach team participating in ongoing community events during throughout the year.
  • Works with CCBHC integrated care team to ensure that the client's behavioral health and addictions treatment, primary/specialty health and social service needs are met through integrated care.
  • Advocate for client to gain access to obtaining services and supports they desire and eliminate barriers.
  • Provides client transportation as necessary.
  • Teams with other staff in co-facilitating psycho-social rehabilitation groups addressing social skills, vocational skills, health and wellness. Administers national evaluation tools as necessary. Participates in performance and quality improvement activities as dictated by supervisor. Administers national evaluation tools as necessary.
  • Participates in ongoing agency supervision, staff meetings, and appropriate training.

QUALIFICATIONS

  • Bachelor's Degree or higher, preferably in a human service field and 2-3 years of pertinent work experience and knowledge of community resources.
  • Knowledge and previous experience serving people with serious mental illness (SMI) and co-occurring disorders. Knowledge and experience with evidence-based practices in behavioral health and addictions treatment. Knowledge and experience in integrated care a plus.
  • Demonstrated enthusiasm, competency, empathy, and warmth. Ability to be firm but fair, encourage self-efficacy and model pro-social beliefs.
  • Flexibility with a high degree of motivation, good judgment, and ability to work independently and as part of an integrated care team.
  • Expertise in a wide range of community based services including trauma informed services, basic motivational interviewing, goal-setting, basic human needs, housing and financial supports.
  • Excellent oral and written communication skills.
  • Proficient in Microsoft environment, notably Word, Excel and Outlook; ability to document services in an Electronic Clinical Record.
  • Written and oral fluency in Spanish strongly preferred.
  • Cultural competence. Must value the differences and integration of cultural attitudes, beliefs, and practices into programming.
