We are seeking a compassionate and dedicated Care Management Coordinator join our team at Pediatric Home Service (PHS). The purpose of this role is to provide comprehensive care coordination for our pediatric clients and their families, ensuring positive outcomes, optimal healthcare utilization, and seamless communication between healthcare providers and community stakeholders

- Hours: The hours are full-time (8a-5p) with some flexibility in scheduling.

JOB DETAILS:
The Care Management Coordinator works closely with Care Managers, families, medical providers, and community partners to help organize and facilitate patient care services. This role ensures seamless communication, supports documentation and compliance requirements, and helps families access the services and resources they need. The ideal candidate is empathetic, highly organized, and passionate about improving care delivery for children with complex medical needs.
Support Care Managers in coordinating services and care plans.
Facilitate communication between providers, families, and community resources.
Schedule care conferences and prepare supporting documentation.
Help families access services like transportation or equipment.
Maintain accurate and up-to-date records in the electronic health record (EHR) system.
Participate in quality improvement efforts and departmental meetings.
Serve as a helpful, knowledgeable liaison for families throughout their care journey.
QUALIFICATIONS:
Education: Associates degree in healthcare, human services, social work, or a related field required. Bachelors degree preferred.
Experience:
12 years of relevant experience in care coordination, home health, customer service, or pediatric healthcare settings.
Experience working with vulnerable or medically complex populations is strongly preferred.
Technical Skills:
Proficiency in Microsoft Office Suite (Word, Excel, Outlook).
Experience with electronic health records (EHR) systems.
Comfortable managing confidential information with accuracy and professionalism.
Communication & Interpersonal Skills:
Strong written and verbal communication skills.
Ability to build rapport with families, providers, and team members.
Strong customer service orientation with a family-centered mindset.
Organizational Skills:
Excellent time management, prioritization, and multitasking abilities.
Capable of managing multiple requests while maintaining accuracy and attention to detail.
Other Desired Qualifications:
Knowledge of community-based services and insurance processes.
Familiarity with HIPAA compliance and medical terminology.
Background in social work, advocacy, or case management support roles.
Experience in homecare, infusion services, or complex pediatric care.
Familiarity with pediatric advocacy groups and disability services.
Comfort working in a fast-paced, mission-driven healthcare environment.
Bilingual (Spanish/English) is a plus but not required.
