Pay range (depending on experience): $80k - $100k/year
Schedule/Shift: Full-Time, Monday-Friday, 8AM-5PM
Territory/Location: Tempe, AZ

Key Responsibilities:
  • Communicates effectively with all members of the health care team and maintains an effective collaborative relationship with the leadership team as well as other departments.
  • Confirms all documentation submitted to workflow is accurate and complete and sufficient to support skilled need and medical necessity.
  • Reinforces agency processes when offering feedback to clinicians regarding documentation deficiencies or OASIS corrections.
  • Make sure visit utilization is supported by individual patient assessment.
  • Confirms clinical documentation supports Admission/Recertification/Discharge decision.
  • Reviews all OASIS assessments for inconsistencies between narrative documentation and OASIS coding, and locks timely.
  • Partners with the Education Department on a variety of quality initiatives recognizing OASIS accuracy is essential to demonstrate improved outcomes.
  • Maintains working knowledge of home health regulations, CHAP standards, conditions of participation and OASIS.
  • Demonstrates the ability to utilize SHP data to improve quality and optimize performance with real-time actionable analytics including managing SHP alerts.
  • Attends Case Conference periodically.
  • Partners with team members to ensure adequate coverage of workflow for each office including on-call rotation to cover holidays and/or weekends if needed.
  • All workflow tasks Must be addressed prior to end of shift.
  • Shows a high level of organizational skills to establish and manage priorities, completing all workflow tasks daily to prevent delays in patient care and assist with timely billing and end of month close.
  • Diligent with follow up on outstanding tasks, showing attention to detail and strong problem-solving abilities.
  • Participates in department meetings as required and adheres to practices outlined by the agency.
  • Additional responsibilities as needed.
Qualifications:
  • Must be a graduate of an accredited School of Physical Therapy
  • Current unencumbered license to practice as a Physical Therapist
  • Minimum of 1 year of home health experience with a working knowledge of Title XXII, Medicare Home Health Conditions of Participation, and OASIS
  • Excellent written and oral communication skills required.
  • Ability to handle stressful situations/deadlines.
  • Strong computer and software skills with knowledge of Home Care Home Base highly preferred.
  • Self-starter, goal directed with ability to work independently and stay focused without on-site supervision.
  • Ability to communicate effectively with agency staff, upper management, and other members of the health care team.
