Position Purpose:

The primary purpose of this position is to provide clinical physical therapy evaluation and treatment for program participants, on acute, restorative or maintenance levels as needed by Day Center, Nursing Home or in-home setting.

What you will do:

  • Conduct an in-person comprehensive physical therapy assessment on all participants upon enrollment and coordinate with the interdisciplinary team to develop comprehensive plan of care.

  • Evaluate, direct, plan, and treat participants with a wide variety of disabilities to help them achieve maximum level of functional independence.

  • Assist in ordering, inventory, distribution and maintenance of durable medical equipment for participants.

  • Assess the need for adaptive Durable Medical Equipment (DME), specialized shoes and prosthesis followed by recommendations to interdisciplinary team; receive authorization prior to purchases and assess all newly issued orthotic shoes, braces and prosthetics for appropriateness and proper fit.

  • Assist as liaison with nursing home and assisted living staff regarding physical therapy needs of participants who are their residents.

  • Evaluate charts and reports on participants' progress, document physical therapy notes in the participants' medical record according to the charting requirements.

  • Assist in recommending physical therapy policies and procedures concerning services provided and maintain quality assurance program, safety, environmental and infection control.

  • Direct, supervise, perform initial and annual competencies, and assist department manager in evaluating the performance of the Physical Therapy Assistant.

  • Train staff in body mechanics, use of equipment, transfers, gait patterns, and methods to maximize safety and functional abilities in performing Activity of Daily Living (ADL).

Minimum Qualifications:

  • Bachelor's degree in physical therapy from an accredited program.

  • Current, unencumbered, active physical therapy license in the state of PACE Organization.

  • One year of experience with a frail or elderly population as a physical therapist, or appropriate training upon hiring.

  • CPR certification and First-aid Certification may be required.

  • Valid drivers license, insurance and use of personal vehicle for each shift due to home care assignments.

  • Must manage conflict well, be customer focused and be able to adapt to ongoing change with regard to work priorities.

  • Superior written and verbal communication skills.

  • Ability to change and be flexible with work priorities.

  • Ability to adapt to changing priorities and work independently.

  • Strong interpersonal, organizational, customer service and problem-solving skills.

  • Proficiency with Microsoft product suite (MS Word, Excel, Power Point, electronic mail and calendaring systems and ability to use other software as required to perform essential functions of the job.
