Apartment Case Manager

Duties and Responsibilities include the following. Other duties may be assigned.

  • Aids admission staff in new client intake and orientation in accordance with New Client Intake Policies and Procedures.
  • Develops, with the full participation of the client, an individualized treatment plan that is pertinent to the clients diagnosis and which addresses clinical progress towards treatment plan.
  • Plans, facilitates and documents client progress towards treatment plan.
  • Assists clients with case management needs such as applying for food stamps, finding treatment programs, helping with employment searches, finding a sponsor.
  • Assists clients to and from treatment groups, individual therapy, program activities and self-help meetings in accordance with daily schedule.
  • Supports program staff with the enforcement of and accountability for rules and guidelines as outlined in handbook.
  • Assists staff and clients in the administration and performance of urine collection and breathalyzer testing in accordance with policies and procedures.
  • Communicates concerning client behavior to appropriate staff in a manner which staff fully understands.
  • Maintains appropriate documentation in EMR system in accordance with SMR policies and procedures.
  • Attends and participates in all trainings, clinical meetings and supervision groups as required by the Clinical Director.
  • Interacts with and supervises clients during onsite leisure and recreational activities in a manner which protects client's confidentiality and supports client care.
  • Assists with the development and revision of Recovery Housing policies and procedures.
  • Supervises resident advisors and ensures they are adhering to program expectations, including SMR Recovery Apartments code of conduct.
  • Performs other duties as assigned by supervisor.

Language Ability:

Ability to read and comprehend simple instructions, short correspondence and memos. Ability to write simple correspondence. Ability to effectively present information in one-on-one and small group situations, clients and other employees of the organization.

Math Ability:

Ability to add, subtract, multiple and divide in all units of measure, using whole numbers, common fractions and decimals. Ability to compute rate, ration and percent and to draw and interpret bar graphs.

Reasoning Ability:

Ability to apply common sense understanding to carry out detailed but uninvolved written or oral instructions. Ability to deal with problems involving a few concrete variables in standardized situations.

Computer Skills:

To perform this job successfully, an individual should have knowledge of word processing software and spreadsheet software.

Certificates and Licenses:

  • CDCA
  • Current TB Test
  • Valid Drivers License

  • Oral and written communication skills
  • Professionalism
