Job Summary

We are seeking a compassionate and skilled Occupational Therapist to join our Home Care team. In this role, you will provide high-quality, patient-centered therapy services to individuals in their homes, helping them regain independence and improve their quality of life.

Key Responsibilities

  • Conduct comprehensive assessments of patients functional abilities, limitations, and rehabilitation needs in their home environment.

  • Develop and implement individualized treatment plans to address patient goals and improve their ability to perform daily activities.

  • Provide education to patients and caregivers regarding adaptive techniques, equipment use, and strategies for maintaining independence.

  • Collaborate with a multidisciplinary team, including physicians, nurses, and other therapists, to ensure cohesive care.

  • Monitor progress, adjust treatment plans as necessary, and document all services in compliance with professional standards and regulations.

  • Ensure safe and effective discharge planning and provide recommendations for additional resources or services if needed.

Minimum qualifications

  • Graduate of an occupational therapy curriculum accredited jointly by the Council on Medical Education of the American Medical Association and the American Occupational Therapy Association.

  • Eligible for the National Registration Examination of the National Board for Certification in Occupational Therapy (NBCOT).

  • Must be able to function in an environment with minimal direct supervision, accepting personal responsibility for maintaining professional relationships with the agencys clients and their families

  • Must accept responsibility for maintaining clinical practice skills, learning and adhering to agencys policies and procedures on an on-going basis.

  • Must have a minimum of one (1) year experience or have a preceptor for a minimum of 6-8 weeks.

  • Home healthcare experience preferred.

  • Must have current drivers license and reliable transportation to and from work site.

  • Ability to consistently demonstrate commitment to the Mission and Organizational Code of Ethics, and adhere to the Compliance Program.
