Physical Therapist PT - Home Health

We are currently seeking a dedicated and compassionate Home Health PT Physical Therapist to join our Home Health team. In this role, you'll help individuals safely regain strength, improve mobility, and maintain their highest level of independence in the comfort of home.

Locations: west Manatee county including Bradenton, Anna Maria Island, Holmes Beach, Bradenton Beach, Palmetto

Schedule: Full-Time; M-F 40 hours with weekend/holiday on-call rotations

Requirements:

• Licensure as a physical therapist in the state of Florida
• Minimum of one (1) year experience in home health care preferred.

What You'll Do:

• Provide physical therapy services to patient as prescribed by a physician in compliance with agency policies and procedures and federal and state regulations.
• Perform initial and ongoing assessments, including OASIS assessments at appropriate time points, to determine level of functioning and patient's ability to perform activities of daily living.
• Develop and revise plans of care, in consultation with the physician and other care team members, which will enable patient to achieve realistic goals of mobility.
• Communicate patient response to treatment and any changes in condition to the physician and designated agency representative.

What You'll Bring:

• Licensure as a physical therapist in the state of Florida
• Minimum of one (1) year experience in home health care preferred.
• Strong clinical assessment skills, clear communication, and a commitment to person-centered care
