This entry-level role involves performing initial assessments, designing treatment plans, and delivering physical therapy interventions for a diverse patient population in an outpatient setting. The position is ideal for recent graduates (DPT required) looking to gain multi-disciplinary experience across outpatient, inpatient, and acute care services. Key Responsibilities: Perform comprehensive initial examinations and develop individualized treatment plans. Implement and modify treatment interventions based on re-evaluations and patient progress. Collaborate effectively with interdisciplinary teams. Complete all required documentation, including evaluations, progress notes, monthly summaries, and discharge reports, in a timely manner. Educate patients, families, and caregivers regarding treatment plans, progress, and home exercise programs. Recommend and document durable medical equipment (DME) needs and write letters of medical necessity when appropriate. Participate in discharge planning and ensure proper follow-up recommendations. Cover additional caseloads as needed based on department staffing. Attend clinical meetings, rounds, and in-service education sessions. Maintain accurate records of therapy sessions, attendance, and patient billing data. Supervise and mentor Rehab Patient Care Associates, students, and volunteers.
