MedAdventures is seeking a travel Home Health Physical Therapist for a travel job in Altoona, Pennsylvania. Job Description & Requirements Specialty: Physical Therapist Discipline: Therapy Start Date: 08/25/2025 Duration: 13 weeks 40 hours per week Shift: 8 hours, days Employment Type: Travel Responsibilities: Evaluate, develop, and implement individualized treatment plans for patients in a home health setting. Complete OASIS assessments and ensure compliance with home health standards. Provide therapeutic interventions to restore function, improve mobility, and reduce pain. Collaborate with referring physicians, healthcare professionals, and team members to optimize patient care. Educate patients and families on exercise programs, injury prevention, and post-treatment care. Maintain accurate and timely documentation of patient progress. Qualifications: Active Physical Therapist (PT) license in PA. Bachelor's, Master's, or Doctorate degree in Physical Therapy from an accredited program.
