Join our team of competent and compassionate professionals as a Counselor-Case Manager in the Connections program at Behavioral Health Network!
Your experience, knowledge and great skills will support the Connections program with the following:
- Performing thorough individual assessments of individuals served.
- Developing individual treatment plans using knowledge of the range of addiction, dual diagnosis, methadone issues, and aftercare planning, as well as the special needs, strengths and weaknesses of individuals served.
- Assessing the outcome of treatment planning, adjusting treatment approaches accordingly, and documenting according to established expectations.
- Providing individual and group counseling sessions to maximize quality and retention in treatment.
- Educating individuals & groups regarding alcohol and drug abuse, harm reduction, mental illness and available community and self-help resources.
- Coordinating referrals for ongoing aftercare treatment with agencies and services and facilitating individual's responsibility and participation in aftercare planning.
- Assisting individuals served toward individual needs regarding resolution of legal issues, homelessness, medication compliance, economic difficulties, employment and continuing care need.
- Providing transportation to individuals served to appointments related to aftercare for medical reasons, as needed.
- Providing care with respect, sensitivity and in a manner that preserves and protects client autonomy, dignity and rights.
Who are we?
The Connections program in Greenfield, MA is a 24-bed residential treatment program serving adult male/nonbinary individuals. Recovery homes provide a safe, structured, and comprehensive rehabilitative environment that supports individuals with their recovery goals after medical and clinical stabilization. Treatment includes care management, group treatment, individualized treatment plans, and peer support.

What are we looking for in a Counselor-Case Manager?
- Commitment to empowerment and growth, human rights, and community integration for individuals served (required).
- High School Diploma or GED equivalent (required); Associate's Degree or higher in related field (preferred).
- At least one year of experience in substance abuse or behavioral health counseling (required).
- Familiarity with Internet, word processing and email systems (required).
- Working knowledge of DPH Bureau of Substance Addiction regulations (required).
- Valid Driver's License (required).
- Vehicle for use during all working hours should be registered and inspected in accordance with 540 CMR 4.00 (preferred).
