HOME HEALTH a PHYSICAL THERAPIST (RPT)We have a great opportunity for aPart TimeHome Health Physical Therapistin Muncie / Alexandria area.1:1 Patient CareFlexibility to Create Your Own ScheduleMileage ReimbursementDrive Time ReimbursementWeekend Hours AvailableService Area a Muncie / AlexandriaJob ResponsibilitiesThe Physical Therapist is responsible for providing rehabilitation treatment and care in the home to the geriatric populationEvaluates patients and develops a rehabilitation plan within the parameters prescribed by the referring physician to improve or restore physical functioning, alleviate pain and prevent physical dysfunctionObserve, record, and report the client's reaction to treatment and any changes in the client's condition to the Physician, DON, and Therapy Services Manager, according to policyPromote coordination of care through interdisciplinary case conferences and communication with physician and field staffDocuments patient progress and communicates with other clinicians such as occupational therapy, speech therapy and skilled nursing on the status of patient's treatment plansParticipate in the agency's quality improvement programPerform therapy interventions utilizing standard physical therapy techniques and skills as appropriate for the condition of the patientDirects and supervises Physical Therapy Assistants on the course of treatment for designated patientsEducates and instructs patients, families, and caregivers regarding treatment plans and proceduresPlans patient discharges and communicates proper follow-up care to patient and caregiversAdheres to established organizational and departmental policies, State and CMS guidelines, standard operating procedures, quality assessment, and safety guidelinesDisplays and maintains behavior which promotes a positive experience for patients, caregivers, referring physicians, and co-workersEducation, Licensure(s), And Qualifications.Experienced Physical Therapist is preferred but not required.New grads welcome to apply!Previous Home Health experience is preferred but not requiredMust possess attention to detail skillsAbility to prioritize work using fundamental time management skillsMust possess excellent interpersonal skills to foster a team environmentDemonstrates knowledge and technical skills necessary for providing competent, comprehensive patient careMust possess problem solving abilities, excellent customer service skills, and patient first characteristicsAbility to perform essential job functions with or without reasonable accommodationMust be currently licensed to practice in the state of IndianaMust be CPR certified or willingness to acquire before start dateMust be a graduate of a Physical Therapy Program accredited by the American Physical Therapy Association (APTA)Current Drivers License, vehicle insurance, and access to a dependable vehicle.Equal Opportunity EmployerPowered by JazzHRdSCkqtYQrp
