Position Summary We are seeking a dedicated and compassionate Physical Therapist (PT) to join our therapy team. This role will primarily serve Nye Legacy in Fremont, NE, with the flexibility to provide coverage at Nye Square in Fremont, NE and, as needed, The Meadows in Norfolk, NE. The PT will evaluate, plan, and implement individualized treatment programs to help patients regain mobility, improve physical function, and enhance overall quality of life.
Key Responsibilities
  • Evaluate patients and develop individualized treatment plans to meet their rehabilitation goals.
  • Provide direct physical therapy treatments including therapeutic exercise, gait training, and functional mobility.
  • Monitor and document patient progress, adjusting treatment plans as appropriate.
  • Collaborate with interdisciplinary team members including physicians, nurses, and occupational/speech therapists.
  • Educate patients and families on treatment goals, safety, and strategies for maximizing independence.
  • Ensure compliance with federal, state, and facility regulations, policies, and procedures.
  • Provide coverage and float between locations as needed to support patient care.
Qualifications
  • Graduate of an accredited Physical Therapy program.
  • Current Physical Therapist (PT) license in Nebraska (or ability to obtain prior to start).
  • Strong clinical skills with a passion for working with older adults.
  • Excellent communication, interpersonal, and documentation skills.
  • Flexibility to float between facilities as needed.
