Home Care Physical Therapist-FT

EveryStep is seeking a compassionate and skilled Physical Therapist to provide high-quality, patient-centered care in the home setting. This role blends hands-on therapy, clinical expertise, and collaboration to help patients regain strength, mobility, and independence.

What You'll Do

• Evaluate patients and create personalized, evidence-based treatment plans
• Deliver direct physical therapy care in patients' homes
• Collaborate with an interdisciplinary care team to support patient goals
• Educate patients, families, and caregivers on treatment plans and home exercises
• Monitor progress and adjust plans of care as needed
• Complete timely and accurate clinical documentation
• Supervise and support PTAs, students, and new staff as appropriate
• Build positive relationships with patients, caregivers, and referral partners

What You Bring

• BS, MS, or DPT in Physical Therapy
• Current Iowa PT license
• Current CPR certification
• Ability to work independently and manage a flexible schedule
• Strong communication skills and a passion for serving diverse populations
• Valid driver's license, insurance, and reliable transportation
