Job Description :

Responsible for performing comprehensive occupational therapy evaluations and delivering individualized treatments based on physician orders. Assesses patient needs, develops and implements therapeutic interventions, and documents care in accordance with regulatory and organizational standards. Collaborates with multidisciplinary teams to enhance patient independence and quality of life. Assists in program development, provides patient and caregiver education, and ensures adherence to policies and procedures. Supports continuous improvement of therapy services to meet diverse patient needs.

Performs comprehensive evaluations and develops individualized treatment plans to address patients physical, cognitive, and psychosocial needs.

Implements evidence-based therapeutic interventions, adaptive equipment, and environmental modifications to enhance patient independence and function.

Assesses patient progress, revises treatment plans as needed, and determines appropriate discharge or continuation of care.

Educates patients and caregivers on home programs, adaptive strategies, and equipment use to promote long-term health and independence.

Collaborates with healthcare team members to ensure coordinated, comprehensive, and patient-centered care.

Instructs rehabilitation technicians, assistants, and students, providing mentorship and oversight.

Maintains accurate, timely documentation and medical records in compliance with regulatory and accrediting standards.

Completes annual competency requirements, maintains licensure, and participates in ongoing professional development.

Assists managerial staff in resource assessment, process improvement, and enhancement of rehabilitation services.

Performs other duties as assigned.

Bachelors degree in occupational therapy or a related field

Masters degree in occupational therapy or a related field is preferred

0-2 years of experience in occupational therapy, rehabilitation, or patient care, or a relevant area

An equivalent combination of education and experience may be considered.

All experience must be paid and in the same related field.

Part-time and PRN experience will be prorated based on hours worked per week.

Volunteer work and internships for academic credit are not counted.

Active Occupational Therapist license in applicable state, Basic Life Support (BLS)

A. Lifting, pushing, and/or pulling objects up to 50lbs: 3. Intermittent (10% - 50% of the time)

B. Lifting, pushing, and/or pulling objects over 50lbs: 2. Occasional ( 50% of the time)

D. Standing or walking with objects up to 25lbs: 3. Intermittent (10% - 50% of the time)

E. Sitting at computer workstation for extended periods: 2. Occasional ( Physical Demands

This position requires regular standing or walking with objects up to 10 pounds; intermittent standing or walking with objects up to 25 pounds; intermittent lifting, pushing, and/or pulling objects up to 50 pounds; occasional lifting, pushing, and/or pulling objects over 50 pounds; intermittent sitting at a computer workstation for extended periods of time; and intermittent performance of repetitive motions (such as documentation or therapeutic activities). Also involves intermittent risk of back injury from moving, lifting, or positioning patients or equipment; occasional risk of injuries from use of equipment; occasional job-related travel; occasional exposure to cuts or punctures and bloodborne or airborne pathogens; and occasional interaction with combative or violent people.
