Home Health Physical Therapist (PT) - Reidsville, NC

Deliver essential physical therapy care directly in patients homes and empower individuals to regain their independence and improve their quality of life. This full-time, 13-week contract offers the opportunity to make a lasting impact by providing patient-centered, evidence-based interventions for those receiving home health services.

Schedule:

• Monday through Friday, 8:00 AM 5:00 PM

What youll do:

• Conduct comprehensive patient evaluations, including detailed OASIS assessments
• Develop and implement individualized plans of care focused on improving mobility, independence, and safety
• Deliver skilled interventions such as therapeutic exercises, gait training, transfer techniques, and fall prevention strategies
• Monitor and document patient progress, making adjustments to treatment plans and communicating with physicians and the care team as needed
• Educate patients and caregivers on exercises, assistive devices, and home safety
• Utilize Home Care Home Base (HCHB) for electronic documentation and ensure compliance with Medicare and agency requirements
• Travel daily to patient residences within your assigned area (mileage reimbursement provided)
• Partner with registered nurses, OTs, SLPs, medical social workers, aides, and participate in case conferences to support optimal patient outcomes

Qualifications:

• Current, active Physical Therapist (PT) license in good standing in state of practice
• At least one year of recent home health PT experience (field experience strongly preferred)
• Proficient in OASIS assessments and documentation
• Familiarity with HCHB preferred
• Strong clinical and organizational skills, able to work independently
• Reliable transportation, active drivers license, and insurance
