Home Health Physical Therapist (PT) - Clifton, CO

Deliver high-quality, patient-centered care as a Home Health Physical Therapist in a dynamic field-based setting. Utilize your expertise in physical therapy to help patients regain strength, mobility, and independence within the comfort of their homes. This is a full-time, 13-week contract opportunity offering a balanced Monday through Friday schedule from 8:00 AM to 5:00 PM, with daily travel to homes in your assigned territory. Expect a caseload of approximately 2532 points per week, typically translating to 57 patient visits per day.

Key Responsibilities:

• Conduct comprehensive physical therapy evaluationsincluding OASIS assessments as required.
• Develop and implement individualized plans of care focused on functional improvement, safety, and pain management.
• Deliver skilled interventions such as therapeutic exercise, neuromuscular re-education, gait and transfer training, and fall prevention.
• Continuously evaluate patient progress, update treatment plans, and communicate with physicians and care team members.
• Instruct patients and caregivers on exercise programs, safety strategies, and effective use of assistive devices.
• Maintain thorough and timely documentation using the Home Care Home Base (HCHB) system.
• Collaborate with interdisciplinary providers including nurses, occupational therapists, and other specialists to ensure holistic care.
• Participate in case conferences, team meetings, and ongoing professional development.
• Uphold agency policies and ensure compliance with all regulatory standards, including Medicare guidelines.

Qualifications:

• Active Physical Therapist (PT) license, in good standing.
• At least 1 year of recent home health physical therapy experience.
• Proficiency in Home Care Home Base (HCHB) preferred.
• Required OASIS assessment experience.
• Reliable transportation, valid drivers license, and auto insurance.
• Strong communication, clinical assessment, and independent practice skills.
