TITLE: PHYSICAL THERAPIST REPORTS TO: Special Education Director JOB GOAL: The Physical Therapist for Ohio County Schools shall offer quality physical therapy for those students for whom such services are required by their IEPs. QUALIFICATIONS: Valid West Virginia State Licensure to practice Physical Therapy ESSENTIAL FUNCTIONS FOR THE JOB: Shall perform reevaluations and execute programs for each client, write re-evaluations and other reports, and report to referring physician. Shall consult, modify, adapt, or advise the purchase of specialized equipment for those students in need of such devices. Shall administer physical therapy as dictated by student's IEP. Shall keep accurate daily notes, write six-month evaluations, and keep files updated. Shall provide support and advice to personnel who come in contact with the student regarding instructional and non-curricular activities. Shall conduct periodic professional learning sessions for the special education department. Shall be present at staff meetings and IEP meetings when schedule allows. Shall act as consultant to other staff members regarding special education students receiving PT services. Shall communicate effectively with students, parents, and staff. Shall be contributing member of the MDET and IEP Committees. Shall maintain records and submits reports for Medicaid eligible services. Shall perform additional duties as may be requested by the Special Education Director. Shall adhere to personnel policies approved by the Board of Directors, and/or Board of Education. Shall be responsible for case management for those cases in which he/she is the primary therapist.
