Home Health Physical Therapist in Fairmont, WV

An opportunity is available for an experienced and compassionate Home Health Physical Therapist (PT) to deliver skilled services to patients in their homes, promoting mobility, strength, balance, and independence. The ideal candidate thrives working autonomously, ensures exceptional patient-focused care, and is adept at collaborating within an interdisciplinary team.

Schedule:

• Full-time, 13-week contract
• Monday Friday, 8:00 AM 5:00 PM

Key Responsibilities:

• Complete thorough home health physical therapy evaluations, including OASIS assessments as needed.
• Develop and implement individualized care plans emphasizing mobility, balance, gait, strength, and safety.
• Deliver therapeutic interventionssuch as exercise programs, gait and transfer training, neuromuscular re-education, and fall prevention.
• Evaluate patient progress, update treatment plans accordingly, and communicate with physicians and other healthcare professionals.
• Educate patients and caregivers on exercise routines, safety protocols, assistive devices, and self-management strategies.
• Accurately document services using Home Care Home Base (HCHB) software.
• Work collaboratively with nurses, occupational therapists, speech therapists, social workers, and aides to provide coordinated care.
• Attend case conferences, team meetings, and participate in continuing education as needed.
• Maintain compliance with agency policies, Medicare, and regulatory standards.

Qualifications:

• Active, unrestricted Physical Therapist (PT) license in the state of practice.
• At least one year of recent home health physical therapy experience preferred.
• Experience with OASIS required.
• Proficiency in Home Care Home Base (HCHB) preferred.
• Excellent clinical assessment, organizational, and communication skills.
• Reliable transportation, valid drivers license, and auto insurance.
• Ability to manage a flexible, travel-based schedule independently.
