Experience:
12 months of experience with 3 months worked in the last 12 months.
Experience with Epic is preferred.
Requirements:
Candidates must have a license (required after submission).
This role will require floating to multiple units
Travel only, local not allowed. Candidates must live >100 miles from facility to be submitted.
COVID vaccination required after submission. Religious and medical declinations accepted.
12 months gap required between for Staff at Program: SimpliFi Program - Monument Health and no current placement allowed at Program: SimpliFi Program - Monument Health.

You can only have 1 active application for the same position (same unit & shift at the same facility). Example: if you apply for day shift, you can also apply for night shift on the same unit and facility, but you cannot apply for another day shift position on the same unit & facility.
Required Skills/Experience: Evaluate and treat a variety of patients from early childhood to end of life. Main focus areas include: speech and language, voice, swallowing/feeding, cognitive-linguistic communication, fluency, augmentative and alternative communication. Applicant will need to know how to complete video fluoroscopic swallow evaluations in the radiology department (again, the swallow study coverage wont be needed)
Shift & Scheduling: Holiday Expectations: Discussed during interview
Floating Requirements: Role primarily in outpatient, floats occasionally to several times per week to hospital for inpatient coverage IP coverage has been minimal, perhaps 1-2 visits / month
7 days maximum time off
2 Manager/Supervisor references required from within the last 12 months
Proof of identification required
