Physical Therapist Home Health - Ankeny, Iowa

An excellent opportunity awaits an experienced Home Health Physical Therapist to provide compassionate, patient-centered care in a field-based setting. In this contract role, you will travel to patients homes, working to promote measurable improvements in mobility, safety, and independence. This position is ideal for a self-motivated clinician who enjoys working autonomously while collaborating with an interdisciplinary team.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM

What Youll Do:

• Complete comprehensive physical therapy assessments, including OASIS start-of-care, resumption, recertification, and discharge evaluations as required
• Create and implement customized treatment plans focusing on mobility, strength, gait, pain management, fall prevention, and safety
• Deliver skilled interventions such as therapeutic exercise, neuromuscular re-education, and transfer or gait training
• Track and document patient outcomes using Home Care Home Base (HCHB); adapt plans according to progress and communicate updates to care teams and physicians
• Provide in-depth education for patients and caregivers regarding exercise programs, assistive devices, and home safety strategies
• Attend case conferences, interdisciplinary meetings, and ongoing educational sessions as needed
• Maintain compliance with agency procedures, Medicare home health regulations, and clinical standards

Desired Experience and Qualifications:

• Active Physical Therapist (PT) license (state-specific, in good standing)
• At least 1 year of recent home health PT experience; field-based care preferred
• Proficiency in HCHB and required OASIS assessment skills
• Reliable transportation, current drivers license, and valid auto insurance
• Excellent communication, time management, and problem-solving skills
• Strong clinical assessment and critical thinking abilities
• Ability to work independently and efficiently manage your schedule
