Schedule:

• Monday Friday, 8:00 AM 5:00 PM
• Full-time, 13-week contract
• Caseload of approximately 2532 points per week, typically 57 daily patient visits
• Travel within an assigned local territory; mileage reimbursement provided

Key Qualifications:

• Valid and active Physical Therapist license in the state
• At least 1 year of recent home health physical therapy experience (field experience valued)
• Experience with OASIS essential; Home Care Home Base (HCHB) proficiency preferred
• Ability to perform clinical assessments and create individualized care plans
• Reliable transportation, current drivers license, and auto insurance
• Strong organizational, communication, and time management skills
• Comfortable working independently and collaborating with an interdisciplinary team

Primary Responsibilities:

• Complete thorough physical therapy evaluations, including OASIS assessments as required
• Develop and deliver customized treatment plans aimed at improving functional mobility, balance, and strength
• Provide skilled therapeutic interventions and patient/caregiver education on safety and home exercise programs
• Monitor and document patient progress in HCHB, adjusting plans as appropriate
• Work closely with nurses, occupational therapists, and other healthcare professionals to ensure well-coordinated care
• Participate in meetings, conferences, and ongoing education to maintain excellence in patient service
