Minimum Requirements
- Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA.
- Active North Carolina Physical Therapy license required to start the assignment.
- BLS certification required from AHA or ARC.
- Minimum of 1 year of Home Health physical therapy experience required.
- Homecare Homebase (HCHB) EMR experience required.
- Experience with geriatric care, chronic disease management, and maintenance OASIS-E documentation required.
- Required skills: gait training, balance training, non-pharmaceutical pain management, plan-of-care development to maximize patient function, effective clinical team communication, and recognition of signs/symptoms to prevent readmissions.
- Valid drivers license and proof of vehicle insurance required. Must be able to drive extended daily mileage (average 6080 miles per day) and meet a 50-mile radius rule from permanent address.
- Preferred: Experience with orthopedics (total hips/knees), CHF, COPD, and post-stroke care. First-time travelers will be considered if experienced in home health.
Assignment Details
- Facility Type: Home Health.
- Shift Name: Days.
- Shift Details: Days 0800-1700.
- Schedule: 5x8, approximately 40 hours per week. Schedule cycles every 13 weeks.
- Patient Population: Adults with chronic disease, orthopedics, post-op hip/knee, fall risk, weakness, CHF, COPD, and occasional stroke.
- Typical daily visits: Approximately 6 visits per day.
- EMR and Technology: Homecare Homebase required. EPIC Rover view-only and Focura for document uploads. Company-issued Samsung Note 9 phone provided for clinical use.
- Orientation: New employee orientation with 5 days of orientation on unit.
- Time-off and Scheduling Notes: No weekend rotation. No on-call. Time off requires manager approval. Staff schedule self-appointments with patients; mileage is reimbursed weekly.
- Travel Requirement: Clinician will travel within the service area including Albemarle, Badin, Harrisburg, Mt. Gilead, Norwood, Troy, and surrounding communities. No maximum daily mileage but average ranges noted above.
