Physical Therapist - Travel Contract

Minimum Requirements

• Bachelor's, Master's, or Doctoral Degree in Physical Therapy from an accredited program approved by the APTA.
• Active Illinois Physical Therapy license required to start the assignment.
• BLS certification from AHA or ARC may be required.
• Outpatient clinic experience required. Experience with evaluations, plan of care development, and functional treatment programs preferred.
• Documentation proficiency required, including goal writing, functional quality indicator assessment, letters of medical necessity, and electronic charge entry.
• Experience supervising and delegating to discipline-specific therapy staff and participating in student clinical education preferred.
• Ability to comply with state practice act, maintain licensure, and complete state-mandated continuing education.
