Home Health Physical Therapist (PT) - Nettie, WV

An opportunity is available for a skilled and compassionate Home Health Physical Therapist (PT) to deliver patient-centered care in a dynamic home health setting. This full-time, 13-week contract role allows you to make a meaningful impact by enhancing mobility, strength, and functional independence for patients in their own homes. Schedule:

• Monday to Friday, 8:00 AM 5:00 PM

Key Qualifications:

• Current and active Physical Therapist (PT) license in good standing (state of practice)
• At least 1 year of recent experience in Home Health Physical Therapy (field experience preferred)
• Proficiency with electronic documentation, especially Home Care Home Base (HCHB) preferred
• Experience with OASIS assessments required
• Strong assessment, clinical decision-making, and patient interaction skills
• Valid drivers license, current auto insurance, and reliable transportation
• Excellent organizational, time management, and communication abilities
• Ability to work independently and manage a field-based caseload

Key Responsibilities:

• Conduct thorough physical therapy assessments, including OASIS documentation, in patient homes
• Design and implement individualized care plans targeting functional and safety goals
• Deliver interventions including therapeutic exercise, gait and transfer training, neuromuscular re-education, and fall prevention
• Educate patients and caregivers on therapy exercises, assistive device use, and in-home safety
• Maintain accurate, timely records within the HCHB system
• Coordinate with nurses, therapists, social workers, and aides to ensure quality care delivery
• Participate in case conferences and adhere to all regulatory and organizational standards
