Schedule/Hours:
- Monday to Friday, 8:00 AM 5:00 PM
- No on-call responsibilities
Key Responsibilities:
- Conduct thorough physical therapy evaluations and implement treatment plans based on physician orders/referrals.
- Use evidence-based assessment tools to establish measurable rehabilitation goals that foster patient independence.
- Monitor progress, reassess goals regularly, and adjust interventions as necessary.
- Collaborate and communicate effectively with the multidisciplinary healthcare team, physicians, patients, and their families.
- Consult on the selection and implementation of adaptive equipment for improved patient outcomes.
- Maintain precise documentation for evaluations, treatments, and billing within deadlines and according to regulatory standards.
- Strictly adhere to organizational, legal, and professional compliance protocols and maintain excellence in care delivery.
- Supervise Physical Therapist Assistants and Aides according to state practice regulations.
- Attend staff meetings, participate in ongoing professional development, and complete required continuing education.
Desired Qualifications:
- Active PT license in Georgia or eligibility through a compact license.
- Two years of experience in home health, preferably using HomeCare Homebase.
- Knowledge of best practices in both hospital-based and home health physical therapy.
- Organization and communication skills vital to balancing clinical documentation and care standards.
- Valid driver's license and ability to travel throughout Lowndes, Echols, Brooks, Cook, Berrien, and Lanier counties.
- Mileage reimbursement provided.
