Position Summary
The Post-Acute Admissions Liaison is responsible to review clinical data, using evidence-based clinical decision support criteria for the referrals for admission to Inpatient Rehabilitation facility. The Post-Acute Admissions Liaison will serve as the point of contact for referral sources, families and patients. The Post-Acute Admissions Liaison will manage the insurance conversations with patient and families as needed.
Qualifications
- Graduate of an AOTA accredited school for Occupational Therapy or Graduate of a CAPTE accredited Physical Therapy program.
- Licensed by the Ohio OT, PT, AT board as an Occupational Therapist or Physical Therapist.
- Obtain and maintain current American Heart Association (AHA) training in Basic Life Support (BLS) utilizing Resuscitation Quality Improvement (RQI) is required.
Skills
- Computer skills with Word and Excel.
- Organized, analytical, and efficient in performance of duties.
- Good oral and written communication skills.
- Must function independently, have flexibility, personal integrity, knowledge of the importance of maintaining confidentiality, and have the ability to work effectively with all levels of hospital personnel.
Responsibilities & Expectations
- Daily collaboration with Hospital Discharge Services and Utilization Management staff members to identify actual and/or potential candidates for post-acute care.
- Collaboration with Aultman post-acute units and leadership team members.
- Communication with medical staff and clinical provider(s) as indicated.
- Review of electronic medical records and clinical documentation for referred patients.
- Review of evidence-based criteria evaluations specific to level of care.
- Face-to-face assessments for referred patients (as required).
- Telephonic and face to face contact with patient, families and/or representatives.
- Completion of required preadmission screening forms for Inpatient Rehabilitation referrals.
- Communication with utilization management and/or specific payer sources regarding pre-authorization and/or verification of insurance benefits.
- Documentation in electronic medical record including, but not limited to Cerner, Ensocare and Careport.
- Maintain data spreadsheet(s) for post-acute care including but not limited to referrals and discharges.
Working Conditions
- Hours- Flexible day shifts Monday - Friday.
- On call every third weekend.
- Subject to frequent interruptions and changes in priority of duties throughout the day.
Hazardous Exposure Category
Category I - Includes tasks that involve exposure to blood, body fluids, or tissues. All procedures or other job-related tasks that involve an inherent potential for spills or splashes of, or mucous membrane or skin contact with blood, body fluids or tissues are Category I tasks. Use of appropriate protective measures is required for every employee engaged in Category I tasks.
