The Physical Therapist (PT) is responsible for examinion, evaluion, diagnosis, prognosis, intervention and the outcomes of the pient/client as described in the New York Ste Physical Therapy Practice Act and Guide to Physical Therapy Practice (APTA). The therapist assists pients to reach their maximum performance and level of functioning, while teaching them to live within the limits of their capabilities. The PT coordines, deleges and supervises approprie responsibilities assigned to support staff (Physical Therapist Assistants, students, rehab aides, etc.). Interacts and practices in collaborion with a variety of interdisciplinary professionals to provide prevention and wellness services including screening and health promotion.

  • Performs a complete system review of the pient
  • Completes accure and approprie test and measures
  • Determines the approprieness for physical therapy trement following the examinion.
  • Recognizes and makes approprie referral to another practitioner as needed.
  • Determines the physical therapy diagnosis based on the physical therapy examinion
  • Determines the amount of time required to reach the optimal level of improvement
  • Develops a plan of care th specifies the interventions to be used and their timing and frequency
  • Conducts a reexaminion to determine changes in pient/client stus and to modify or redirect interventions as approprie
  • Uses various physical therapy procedures and techniques to produce changes in the condition th are consistent with the diagnosis and prognosis
  • Observes and evalues trement effect, and makes changes to the plan of care as needed
  • Performs all aspects of pient care in an environment th optimizes pient and therapist safety and reduces the likelihood of medical/health care errors
  • Performs pient/ family educion timely to prevent delays in discharge
  • Demonstres the ability to tre pients and their families with respect and dignity.
  • Coordines and directs pient care to ensure pients needs are met and hospital policy is followed.
  • Determines the anticiped goals and expected outcomes for the episode of care
  • Participes in outcomes da collection and analysis to determine the effectiveness of intervention
  • Maintains accure and timely documention th meets current standards and policies
  • Interacts professionally with pients/ families and involves the pient/ family in the formion of the plan of care
  • Maintains current pient schedule board or book
  • Consults other departments as approprie to collabore in pient care and performance improvement activities
Educion/Experience Requirements

REQUIRED:

  • Gradue of Bachelor of Science program in Physical Therapy with curriculum approved by the American Physical Therapy Associion.
  • New York Ste Physical Therapist licensure and registrion.
  • One-year experience as a staff physical therapist under qualified supervision.

PREFERRED

  • DPT or Masters degree in Physical Therapy.
Licensure/Certificion Requirements

  • New York Ste Physical Therapist licensure and registrion.
