Shift: 8:30 AM - 5:00 PM

Experience:

12 months of experience with 3 months worked in the last 12 months.

Experience with Cerner is preferred.

Requirements:

Candidates must have a license (required for submission).

Local & travel allowed.

12 months gap required between for Staff at Program: Aya Program - Memorial Health System - IL and no current placement allowed at Program: Aya Program - Memorial Health System - IL.

12 months gap required between for Per Diem at Program: Aya Program - Memorial Health System - IL and no current placement allowed at Program: Aya Program - Memorial Health System - IL.

Additional Details:

References must cover at least 1 full year of relevant experience in specialty within the past 3 years.

Unit Details:Pelvic health outpatient physical therapy role seeing male and female pelvic floor patients.Adolescent, adult, and geriatric patient population.

Special Requests:RTO: Ideally no more than 5 days.Allied clinicians that do not require a state license in hand must arrive 3 days prior to start for fingerprints.You must be able to complete labs within 48 hours of signing.

Shift & Scheduling:Schedule is M-F.No weekends.Open to 13-26 Week contracts

5 days maximum time off. Facility holidays: 11/26/2026, 12/25/2026, 01/01/2027. Number of holidays allowed off: 2

1 Manager/Supervisor reference required

1 Charge/Lead reference required
