Minimum Requirements

  • Doctoral, Masters, or Bachelors Degree in Physical Therapy from an accredited program approved by the APTA.
  • Current, unrestricted WV state license or ability to obtain state licensure prior to start.
  • BLS certification from AHA or ARC may be required.
  • Experience with OASIS visits and home health documentation preferred.
  • Home health experience preferred.
  • Ability to perform add-on evaluations as required by the client.
