- The Home Care Physical Therapist at EHM Senior Solutions is responsible for providing high-quality physical therapy services to patients in their homes. This role is crucial in helping patients regain mobility, improve physical function, and achieve their rehabilitation goals. The therapist will work collaboratively with a multidisciplinary team to ensure comprehensive care and optimal patient outcomes.

Key Responsibilities:

- Conduct thorough assessments of patients' physical conditions and develop individualized treatment plans.

- Implement therapeutic exercises and interventions to improve patients' mobility, strength, and coordination.

- Educate patients and their families on exercises and activities to be performed at home to enhance recovery.

- Monitor and document patients' progress, adjusting treatment plans as necessary to achieve desired outcomes.

- Collaborate with physicians, nurses, and other healthcare professionals to ensure coordinated and effective patient care.

- Maintain accurate and timely records of patient visits, treatments, and progress notes.

- Adhere to all professional, ethical, and regulatory standards in the delivery of physical therapy services.

- Participate in ongoing professional development to stay current with advancements in physical therapy practices.QualificationsRequired Education:

- Doctorate in Physical Therapy from an accredited institution.

- Must possess an active license in the State of Michigan in good standing.

Required Experience:

- Minimum of 1 year of experience as a licensed Physical Therapist, in a home health setting.

- Proven track record of providing high-quality physical therapy services on a per-visit basis.

Required Skills and Abilities:

- Strong knowledge of physical therapy principles, techniques, and equipment.

- Ability to assess patient needs and develop effective treatment plans.

- Excellent communication skills for interacting with patients, families, and other healthcare professionals.

- Strong organizational skills to manage multiple patient visits efficiently.

- Ability to work independently and make clinical decisions in a home or community setting.

- Proficiency in maintaining accurate patient records and documentation.

- Demonstrated ability to adapt therapy techniques to meet the unique needs of elderly patients.

- Experience with Wellsky/Kinnser and Forcura preferred

Work Days/ Hours: Contingent Position

Any day Monday - Friday, set own hours

Paid travel time, paid mileage and pay per visit rates

3-4 admissions/week

On-Call Rotation: Yes

1 summer holiday, 1 winter holiday and 1 weekend a month
