Reports To: Assistant Director of Health Services
Employment Type: Full-Time, Non-Exempt
Starting Wage: $25-30 per hour, commensurate with experience

Your Core Responsibilities:
- Provide occupational therapy services under the supervision of a Registered Occupational Therapist (OTR) or Physical Therapist (PT).
- Assist in developing and implementing individualized OT/PT service plans.
- Complete therapy assessments, progress notes, monthly summaries, and required clinical documentation.
- Collaborate with interdisciplinary teams to support therapeutic goals and services.
- Provide education and training to staff regarding therapy-related supports and interventions.
- Identify and request adaptive equipment and therapy-related supplies.
- Monitor individual progress and communicate updates or concerns to supervising clinicians.
- Ensure compliance with agency policies, clinical standards, and regulatory requirements.
What You Bring:
- Certification as an Occupational Therapy Assistant through the New York State Department of Education required.
- Minimum of one (1) year of related experience preferred.
- Valid Drivers License, reliable transportation and ability to meet Agency Motor Vehicle Operating Requirements (MVOR).
- Strong written, verbal, and interpersonal communication skills.
- Ability to work collaboratively within an interdisciplinary team environment.
- Ability to provide hands-on support and therapeutic interventions.
- Ability to lift up to 35 pounds.
- Ability to meet SCIP and/or agency vehicle operator requirements.
