Physical Therapist - Skilled Nursing Facility | Mauston, WI

We are seeking a dedicated and compassionate Physical Therapist (PT) for a permanent, full-time opportunity at a well-established medical center in Mauston, Wisconsin. Position Overview: The Physical Therapist will play a key role in helping patients regain mobility, manage pain, reduce fall risk, and improve their overall independence and quality of life within a Skilled Nursing Facility (SNF) setting.

Key Responsibilities:

Evaluate and treat patients experiencing movement dysfunction or pain to improve their ability to perform daily activities

Develop and implement individualized treatment plans focused on patient recovery and functional improvement

Assist patients in post-surgical recovery, fall prevention, and DVT risk reduction

Maintain thorough and timely documentation including evaluations, progress notes, daily notes, and discharge summaries

Coordinate with physicians, insurance companies, nursing staff, and other rehabilitation team members

Participate in discharge planning to ensure safe patient transitions

Requirements:

Graduate of an accredited Physical Therapy program

Active Wisconsin PT license (or eligibility to obtain upon acceptance)

Less than 1 year of experience accepted new graduates welcome!

Strong communication, documentation, and interpersonal skills

Commitment to patient-centered care

Compensation & Benefits:

Competitive salary: $91,000 - $138,000 annually (based on experience)
