Key Qualifications:

• Valid Physical Therapist license required
• At least one year of Home Health experience required
• HCHB experience preferred, but not required
• Strong interpersonal, communication, and documentation skills

Primary Responsibilities:

• Conduct physical therapy evaluations and treatments based on provider orders/referrals
• Utilize appropriate assessment tools to craft comprehensive and individualized treatment plans
• Review records to ensure safe and effective care delivery
• Set measurable goals to maximize patient function and independence
• Routinely re-assess and adjust treatment plans as needed
• Maintain clear communication with physicians, treatment teams, and families
• Recommend/adapt special equipment to enhance rehabilitation outcomes
• Complete and submit documentation promptly to maintain compliance
• Supervise PTAs and Therapy Aides within scope of practice
• Participate in staff development meetings and pursue ongoing clinical education
