Qualifications/Required Experience:

• Graduate of an accredited physical therapy program
• Unencumbered, current physical therapy license for the state of assignment
• 23 years physical therapy experience preferred
• Current CPR certification
• Familiarity with computer navigation, email, and self-management of online HR/training platforms
• Reliable transportation, drivers license, and required liability insurance

Key Responsibilities:

• Complete patient evaluations and provide therapeutic services as ordered by physicians
• Assist with development and updates to treatment plans
• Observe, record, and report on patient condition and progress
• Supervise and evaluate Physical Therapy Assistants and Home Health Aides as assigned
• Advise and educate patients, families, and agency staff regarding therapy programs
• Ensure compliance with state-specific regulations regarding therapy services and supervision
• Participate in in-service training and periodic office meetings
• Assess home environments and provide recommendations for therapy or safety
