Speech Language Pathologist (SLP) - Home Health

Wages Up to $50/hr based on experience

Summary/Objective

The speech therapist evaluates, diagnoses, and treats patients with speech, language, voice, fluency, and swallowing disorders within the comfort of their homes.

Patient Care

  • Completes a comprehensive assessment of patients' speech, language, and swallowing abilities.
  • Develop and implement personalized therapy plans based on each patient's needs and goals.
  • Educate patients, family members, and caregivers on therapeutic exercises and communication strategies.
  • Use evidence-based techniques to treat speech, language, cognitive-communication, and swallowing disorders.
  • Collaborate with other members of the healthcare team, including physical therapists, occupational therapists, and nursing staff to ensure a holistic approach to patient care.
  • Complies with agency policies and procedures. Promotes and maintains an agency environment in compliance with federal, state, and local regulations.
  • Regularly evaluate and document patient progress, adjusting therapy plans as needed.
  • Provide guidance on communication devises and techniques as appropriate for individual patient needs.
  • Maintain detailed, timely documentation in compliance with agency, state, and federal regulations.

Communication

  • Prepares clinical notes and updates the primary physician when necessary and at least every 60 days.
  • Communicates with the physician regarding the patient's needs and reports any changes in the patient's condition; obtains/receives physician's orders as required.
  • Communicates with community health related persons to coordinate the care plan.
  • Ability to work independently and manage a flexible schedule.

Additional Duties

  • Ensures that arrangements for equipment and other necessary items and services are available.
  • Attend in-service training and mandatory agency meetings.
  • Maintains a professional appearance.

Work Environment

The responsibilities of this position involve significant physical activities including standing, lifting, bending, stooping, pushing, pulling and twisting. The employee must be a licensed driver with an automobile that is in good working order and insured in accordance with the organization requirements. The employee may experience a change in work conditions related to weather issues, traveling in rain, snow, etc., and must be able to adapt to the work environment to meet patient care needs.

While performing the duties of this job, the employee is regularly required to talk or hear. The employee frequently is required to stand; walk; use hands to finger, handle or feel; and reach with hands and arms.

Education and Experience

  • Possesses a degree from a Master's program in Speech - Language Pathology from an accredited program.
  • Current SLP licensure in state and Certificate of Clinical Competence from ASHA
  • Two years of appropriate experience as a speech therapist. Community/home health experience preferred
  • Excellent observation, verbal and written communication skills, and problem solving skills
  • Knowledge of speech therapy software and adaptive communication technology
  • Bilingual proficiency.
