Minimum Requirements
- Associate's, Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA. (Client minimum: Associates Degree accepted.)
- Active Kentucky Physical Therapist license required to start the assignment.
- Home health experience required. Must be proficient with HCHB electronic documentation.
- Experience performing OASIS assessments, admissions, recertifications, 30-day assessments, and discharges.
- Experience supervising PTAs.
- BLS certification from AHA or ARC may be required.
- Preferred: prior experience working in Logan and Todd county coverage areas.
Assignment Details
- Facility Type: Home Health
- Shift Name: Days
- Shift Schedule: Monday - Friday, approximate daytime schedule (am to 4:30 pm) as indicated by the client.
- Weekly Guarantee: 32 hours per week.
- Productivity Expectation: 30 points.
- Coverage Areas: Logan County and Todd County, KY.
- Mileage reimbursement between patient visits from first to last patient: Yes.
- On-call/Weekend Requirement: No.
- Dress Code / Scrub Color: Royal blue.
- Floating Requirement: Candidate may be required to cover assigned geographic areas as needed.
