Physical Therapist - Travel Contract

Minimum Requirements

• Bachelor's, Master's, or Doctoral Degree in Physical Therapy from an accredited program approved by the APTA.
• Active Iowa Physical Therapy license required to start the assignment.
• BLS certification from the American Heart Association or American Red Cross may be required.
• Home health experience preferred.
• Experience with electronic medical records preferred; Epic experience preferred but not required.
• Ability to travel within a defined geographic region and provide care in patients' homes as assigned.

Preferred Qualifications

• Previous home health caseload management experience.
• Experience working with post-acute, geriatrics, or chronic condition populations.
• Comfortable performing documentation and communication remotely when required.

Assignment Details

• Shift hours include 08:0016:00 (also may include 09:0017:00 option).
• 40 hours per week.
• Weekend coverage as needed.
• May require floating within the regional home health network.

Impacting Patient Care

Provide in-home physical therapy to support patient independence and functional recovery. Coordinate with interdisciplinary teams to implement individualized plans of care. Document clinical progress and discharge planning in the electronic medical record to support continuity of care.
