As a small, independently owned practice located in Lexington, KY, we offer a close-knit team environment where every staff member plays a meaningful role in patient outcomes. We are seeking a dedicated and licensed Physical Therapist Assistant to join our collaborative team.

Daily Responsibilities

• Implement individualized treatment plans under the supervision and direction of a licensed Physical Therapist
• Deliver therapeutic interventions including therapeutic exercise, manual therapy, and modalities to improve patient outcomes
• Collaborate with the clinical team to ensure comprehensive, patient-centered care
• Participate in care planning and discharge planning processes
• Maintain accurate and timely documentation in compliance with regulatory and payer requirements

Required Qualifications

• Active PTA license in the state of Kentucky
• Minimum 2 years of experience as a Physical Therapist Assistant
• Outpatient rehabilitation experience
• Strong documentation skills and knowledge of good documentation practices
• Experience with treatment planning and plan of care implementation

Preferred Qualifications

• Skilled nursing facility experience
• Experience with a broad range of therapeutic interventions and exercise programming
