Job Description
Looking for a change of scenery and a chance to help others. Provide skilled home health physical therapy services to patients in Bluefield, WV and surrounding counties. The clinician will perform evaluations and skilled treatment based on physician orders, develop measurable goals and treatment plans, reassess progress, document timely and accurately, communicate with the interdisciplinary team and family, recommend and facilitate adaptive equipment, and supervise PT assistants and aides as allowed by state practice act. Maintain compliance with HIPAA, company policies, documentation and billing requirements, and applicable state and federal laws.
Minimum Qualifications
- Bachelors, Masters or Doctoral degree in Physical Therapy from an accredited program recognized by the APTA.
- Current West Virginia physical therapy license required prior to start.
- Prior home health physical therapy experience required.
- Experience with Home Care Home Base (HCHB) electronic documentation system required.
- Demonstrated ability to complete comprehensive evaluations, develop measurable goals, and document treatment in required formats and timelines.
- Ability to supervise PT Assistants and Physical Therapy Aides in accordance with state practice act.
- Basic Life Support (BLS) certification from American Heart Association or American Red Cross may be required.
- Ability to travel within assigned counties, including Mercer and McDowell counties, as part of the caseload.
- Productivity expectation: ability to meet 30 productivity points as required by the client.
Preferred Qualifications
- Experience securing and fitting adaptive equipment for patients in home settings.
- Prior experience with documentation and billing processes specific to home health and thirdparty payers.
- Experience working with a broad adult patient population and common comorbidities seen in home health.
Assignment Details
- Shift hours: 8:00 AM to 5:00 PM.
- Work days: Monday through Friday.
- No on-call requirement.
- Counties covered: Mercer and McDowell counties; travel to patient homes within these counties required.
- Mileage reimbursement provided at client specified rate.
