Shift: 8:00 AM - 5:00 PM

Experience:

24 months of experience with 3 months worked in the last 12 months.

Experience with Epic is preferred.

Requirements:

Candidates must have a license (required for submission).

This role may require floating to additional units and locations

Travel only, local not allowed. Candidates must live >60 miles from facility to be submitted.

12 months gap required between for Staff at Program: Medical Solutions Plus Program - (Mountain) CommonSpirit Health and no current placement allowed at Program: Medical Solutions Plus Program - (Mountain) CommonSpirit Health.

12 months gap required between for Per Diem at Program: Medical Solutions Plus Program - (Mountain) CommonSpirit Health and no current placement allowed at Program: Medical Solutions Plus Program - (Mountain) CommonSpirit Health.

Additional Details:

An interview may not be available prior to offer.At least one reference must be from the same unit/specialty as the job youre applying to.

Special Requests:Cannot have worked at any CommonSpirit location within the last 12 months.Cannot apply to more than one job order on the same unit and shift at the facility.

Unit Details:45 minute 1:1 treatments; evaluation and recurring schedule of 9 patients; management of cancellations/no-shows is imperative.Small community outpatient rehab with mostly orthopedic patients and a wide variety of cases.PTAs are available; treatment model is 1:1.

Shift & Scheduling:

26 week contract only.Monday-Friday.No weekends or holidays.Must be in state for the week of orientation, even if first-day modules are remote.

Floating Requirement: required to cover outpatient

Facility holidays: 11/26/2026, 12/25/2026, 01/01/2027. Number of holidays allowed off: 1. Start assignment restriction: 14 days

1 Charge/Lead reference required from within the last 12 months

1 reference required from within the last 3 years

Drivers license required for submission
