VNA & Hospice of the Southwest Region is seeking compassionate and motivated Physical Therapists (PTs) to join our team in Rutland County. We are hiring for full-time and per diem positions working with adult and/or pediatric patients in the home health setting.

POSITION SUMMARY

As a Physical Therapist, you will evaluate patients, develop individualized treatment plans, and provide skilled therapy services in the home, facility, or educational environment. Caseloads may include adult and/or pediatric patients depending on experience and interest.

Responsibilities include:

Completing PT and OASIS assessments as appropriate

Developing and implementing individualized plans of care

Providing therapeutic interventions and caregiver education

Monitoring patient progress and adjusting treatment plans as needed

Collaborating with interdisciplinary team members

Supervising PTAs, students, and home care aides when applicable

Maintaining timely and accurate clinical documentation

QUALIFICATIONS

Degree in Physical Therapy from an accredited program

Current Vermont PT license

Valid driver's license, ability to successfully pass pre-employment driver record, criminal, adult and child abuse background check/review required.

Home health or prior clinical experience preferred

Ideal candidates are compassionate, organized, flexible, and comfortable working independently while remaining part of a collaborative care team.
