Minimum Qualifications
- Associate degree in Physical Therapy from an accredited educational program.
- Active Ohio state PTA license required to start the assignment.
- BLS certification from AHA or ARC may be required.
- Minimum 12 months of Home Health experience within the past 3 years (client requirement).
- Experience with HCHB and OASIS documentation systems (client requirement).
Preferred Qualifications
- Previous travel or agency home health experience.
- Familiarity with productivity expectations in home health settings.
Assignment Details
- Shift schedule: Days, 8-hour shifts, Monday through Friday.
- No on-call responsibilities.
- Productivity expectation: 32 points per week (based on a 40-hour week).
- EMR systems: HCHB and OASIS required.
- Candidate may be assigned within the local territory as needed; floating to other nearby service areas is possible.
