Job Description & Requirements Specialty: Physical Therapist Discipline: Therapy Duration: 11 weeks 40 hours per week Shift: 8 hours, days Employment Type: Local Contract Pride Health is hiring a Physical Therapist to support our client's patients in Summersville, WV, with coverage in Fayette and Nicholas Counties. This is an 11-week assignment (Possibility of extension or conversion) and a great way to start working with a top-tier healthcare organization! Job Details: Job Name: Home Health Physical Therapist Contract type: Local Contract Location: Summersville, WV (Coverage Area: Fayette and Nicholas County) Duration: 11+ week Contract (Possibilities of extension) Pay rate: $50-56/hr. On W2 (Negotiable - Pay Rate is based on experience) Schedule: Mon - Fri (5 days/wk.) from 8 AM to 4:30 PM (32 hrs. guaranteed) On Call/ weekend Requirement: To be discussed with HM during the interview . Mileage: Reimbursement provided (from first patient to last patient) Job Responsibilities: Perform Start of Care (SOC) visits, completing initial assessments and establishing individualized Plans of Care (POC) in accordance with physician orders. Conduct thorough physical therapy evaluations assessing strength, mobility, balance, pain, and functional limitations. Provide skilled therapy interventions during routine and follow-up (revisit) appointments. Perform re-evaluations to assess patient progress, update treatment plans, and adjust goals as needed. Develop and implement evidence-based treatment plans tailored to each patient's needs and goals. Educate patients and caregivers on home exercise programs, safety precautions, and disease management. Complete timely and accurate clinical documentation in compliance with regulatory and organizational standards. Coordinate care with physicians, interdisciplinary team members, and caregivers to ensure optimal patient outcomes. Conduct discharge visits, including final assessments, patient education, and transition planning. Qualifications: Graduate of an accredited Physical Therapy program (master's or higher). Active, unrestricted Physical Therapist (PT) license in West Virginia. Minimum 1 year of clinical experience (home health experience required). Valid driver's license and reliable transportation.
