This is a flexible PRN opportunity designed for clinicians who value autonomy, meaningful patient connections, and the ability to see the direct results of their work.
What You’ll Do- Deliver hands-on occupational therapy interventions in patients’ homes under the guidance of a licensed Occupational Therapist
- Help patients regain independence with activities of daily living (ADLs) like dressing, bathing, and mobility
- Track progress and provide real-time feedback to adjust care for the best outcomes
- Educate patients and families on adaptive techniques and equipment that make everyday life easier
- Maintain accurate, timely documentation to ensure continuity of care
- Collaborate with a multidisciplinary team committed to exceptional patient outcomes
- Active Certified Occupational Therapy Assistant (COTA) license
- Current CPR certification
- Valid driver’s license & auto insurance
- Ability to work independently across different home environments
- Strong communication skills and a patient-first mindset
Preferred:
- Home health or SNF experience
- Familiarity with HomeCare HomeBase (HCHB)
