Minimum Qualifications
- Associate degree in Physical Therapy from an accredited program.
- Active Indiana PTA license required to start the assignment.
- BLS certification from AHA or ARC may be required.
- At least 12 months of home health experience within the past 3 years (client requirement).
- Experience with HCHB (HomeCare HomeBase) EMR required.
- Ability to manage a schedule of approximately 30 visits per week, territory covering Indianapolis proper (Teams 34).
Preferred Qualifications
- Prior travel or contract home health experience preferred.
Assignment Details
- Shift: 8-hour days, Monday through Friday.
- Schedule: MondayFriday, approximately 30 visits per week.
- Territory: Indianapolis proper (Teams 34).
- Call: No on-call required.
