Physical Therapy Assistant - Travel Contract

Minimum Qualifications

• Associate degree in Physical Therapy from an accredited program.
• Active Indiana PTA license required to start the assignment.
• BLS certification from AHA or ARC may be required.
• At least 12 months of home health experience within the past 3 years (client requirement).
• Experience with HCHB (HomeCare HomeBase) EMR required.
• Ability to manage a schedule of approximately 30 visits per week, territory covering Indianapolis proper (Teams 34).

Preferred Qualifications

• Prior travel or contract home health experience preferred.

Assignment Details

• Shift: 8-hour days, Monday through Friday.
• Schedule: MondayFriday, approximately 30 visits per week.
• Territory: Indianapolis proper (Teams 34).
• Call: No on-call required.
