Physical Therapist - Travel Contract

Minimum Requirements

• Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA.
• Active Utah State Physical Therapy license required to start the assignment.
• BLS certification from AHA or ARC may be required.
• Experience working in a skilled nursing facility preferred.
• Ability to provide outpatient and restorative care in a long-term care environment preferred.
• Must be able to work a 5-day weekly schedule consistent with facility staffing needs (32 hours guaranteed as noted by client).

Preferred Qualifications

• Prior experience as a travel physical therapist preferred.
• Experience with common SNF documentation systems preferred.
• Experience managing caseloads in post-acute or skilled nursing settings preferred.

Assignment Details

• Schedule: 5 days per week, 32 hours guaranteed as noted by the client.
• Shift pattern: 4x8 or 5x8 schedules referenced by client; exact daily hours to be confirmed with recruiter.
• Location: Brigham City, UT. Candidate may be expected to float to nearby facilities as operational needs require.
