What Youll Bring:

• Active PTA license in the state of practice (in good standing)
• At least 1 year of recent Home Health experience as a PTA (field-based preferred)
• Familiarity with Home Care Home Base (HCHB); OASIS knowledge preferred
• Strong assessment, observation, and communication skills
• Self-motivation and ability to work independently
• Reliable transportation, valid drivers license, and auto insurance
• Excellent organizational and time-management abilities

What Youll Do:

• Carry out skilled physical therapy treatments based on care plans created by the supervising PT
• Assist patients with therapeutic exercises, gait/balance training, and functional mobility tasks
• Monitor, document, and report on patient progress to ensure continuity of care
• Communicate updates and recommendations to the therapy and care team
• Educate patients and caregivers on safety, assistive device use, and at-home exercise plans
• Maintain accurate documentation in Home Care Home Base (HCHB)
• Collaborate within a multidisciplinary healthcare team and participate in case conferences
• Adhere to professional and agency standards as well as Medicare home health guidelines
