Physical Therapist
Top Responsibilities:
* Collaborate with interdisciplinary team members including nurses, PTAs, occupational therapists, and home health aides regarding patient care, updates, and concerns.
* Develop, implement, and follow individualized treatment plans in coordination with physicians and care teams.
* Provide in-home physical therapy services focused on improving patient mobility, strength, and overall functional independence.
* Monitor patient progress, reassess conditions, and modify care plans as needed.
* Maintain accurate and timely documentation of patient treatments and progress in accordance with company policies and regulatory standards.
* Participate in weekly case conferences and contribute to care planning discussions.
* Educate patients and caregivers on therapeutic exercises, safety techniques, and home care plans.

Required Qualifications: * Active Physical Therapy license * Master's or Doctorate degree in Physical Therapy * Minimum 1+ year of experience in hospital, SNF, or outpatient settings (home health experience preferred)
